DESCRIPTION:tobm \n  \nLIFTT invites individuals experiencing Diabetes\, High Blood Pressure\, Chronic Kidney Disease\, and Heart Failure to join us for a FREE course called the DECIDE Self-Management Program. The classes will be held from 12:00 p.m. to 1:00 p.m. on Fridays\, from October 10 to December 5\, 2025 (no class on Friday\, November 28). To make DECIDE available to our entire service area\, the sessions will be Hybrid\, with the in-person classes being held at the Billings LIFTT Office (1241 Crawford Drive) and live-streamed using Microsoft Teams. \nDesigned with funding from the National Institutes of Health\, DECIDE is an evidence-based self-management support program designed to help participants tackle real-life barriers and improve their health outcomes.
