LIFTT invites those experiencing Diabetes, High Blood Pressure, Chronic Kidney Disease, and Heart Failure to join us for a FREE nine-session course called the DECIDE Self-Management Program. The classes will be held from 12 p.m. to 1 p.m. each Friday between September 12 and November 7. To make DECIDE available to our entire service area, the sessions will be Hybrid, with the in-person classes being held at the Billings LIFTT Office (1241 Crawford Drive) and live-streamed using Microsoft Teams.
Designed through funding by the National Institutes of Health, DECIDE is an evidence-based self-management support program that is designed to help participants tackle real-life barriers to improve their health outcomes. Topics covered by the DECIDE program include:
