Diabetes Prevention Program

National Diabetes Prevention Program:

The T2 curriculum, also known as the PreventT2 curriculum, is a 12-month lifestyle change program developed by the Centers for Disease Control and Prevention (CDC) to prevent type 2 diabetes. Participants are encouraged by Lifestyle coach facilitators to make small habit changes that promote modest weight loss and improve nutrition. They receive continual peer support and are able to monitor their progress by analyzing their metabolic labs at the beginning and the end of the program.

If you receive Medicaid benefits, you may qualify for a $100 food box for attending this class. Please contact us to determine if you are eligible!

SAIL

SAIL, or Stay Active and Independent for Life, is an evidence-based program designed for adults 65 and older to reduce the risk of falls and improve overall well-being. It focuses on improving strength, balance, and fitness through a structures series of exercises that can be done either standing or sitting.

Walk with Ease:

The Arthritis Foundation’s Walk With Ease Program is a community-based physical activity and self-management education program. While walking is the central activity, Walk With Ease is a multi-component program that also includes health education, stretching and strengthening exercises, and motivational strategies. The program includes a guidebook and a walking schedule to get you safely moving toward better health.

Studies by the Thurston Arthritis Research Center and the Institute on Aging of the University of North Carolina show that Walk With Ease is proven to:

  • Reduce the pain and discomfort of arthritis
  • Increase balance, strength and walking pace
  • Build confidence in your ability to be physically active
  • Improve overall health

The Walk With Ease program is offered as a self-guided course or in a community setting. Choose the format that best suits you.
